How to Claim?

Mental Health Coverage
Your Mental Health Coverage: $500 per benefit year combined for a Psychologist, Master of Social Work, or Psychotherapist.
Covered by GreenShield.
All psychologists (PhD), Masters of Social Work (MSW), and Registered Psychotherapists (RP) licensed to practice in Ontario will be covered by your Greenshield insurance.
Register with Greenshield
1. Log in as plan member
Log in at:https://www.greenshield.ca/en-ca/)
2. Get a registration Key
Plan Member ID: UTG(student number)-00
Example: UTG1111111111-00
Replace -00 with -01 or -02 for number of dependents
Enter your first name & surname to receive your registration key
Submit Your Claim
1. Log in to your Greenshield account
2. Click on ‘Submit a Claim’
3. Select ‘Professional Services’ and pick the service you accessed
4. Search for your provider: enter your psychologist, social worker, or psychotherapist name & phone number
- Their information can be found on receipt from your session
5. Enter information about your session: date, length of session, total amount for visit, etc.
6. Submit!
Mail in your Claim Forms
1. Download ‘Health & Vision Claim Form’
2. Fill in the form
Your Plan Member ID: UTG(student number)-00
3. Include receipt from your session
Receipt from your counsellor should include your name, date and session type, and charge for session.
